What's wrong with how this coffee was brewed?

Hi all,

My office has consistently been brewing questionable coffee. Our supplier has even come in and given a "how to brew" demo session to the office management several times, however it still looks very questionable and tastes off. Do any of you know what may be causing our coffee to look like it does below?

The below coffee has just been sitting for a few minutes - it was not drank from at all. There were a lot of bubbles at the top when it was first poured. You can see there is a line of residue where the bubbles were and there are still some just sitting atop the coffee.

Any insight is appreciated. Thank you.

The first place to always look is "What coffee are you brewing???" (Who roasts it?)

It all begins with the coffee. And if it's over roasted, stale (older than 2 weeks from roast date), or worse, pre-ground.... then there is no saving it.
 
Check your water. Do you use "soft" water where sodium may be the culprit. Ideal hardness for coffee is around 130-150ppm. Also, I've read that where the composition of minerals leans toward magnesium, it brightens the natural acidity of the coffee. Also check the pH of the water as the precipitation of solids is likely the culprit for the residue!!
 
Pour quality and stale coffee, too fine of a grind, and a dirty machine all come to mind.
